Your name, address, phone number, and email. A sworn declaration under penalty of perjury that you believe the content was removed in error. A declaration consenting to legal action and agreeing to accept legal documents. Your signature, either physical or electronic. After receiving a valid counter-notice, we may restore the content unless the complainant takes legal action.
